A teenager has died by suicide in a Western Australian youth detention centre, becoming the second child to die in custody in the state in less than a year.
Commissioner of Corrective Services Brad Royce said the teenager had only arrived at the detention centre two days before he died and was out of his cell for most of yesterday afternoon.
Mr Royce said staff checked on the boy over the course of the evening 10 times and on the 11th occasion he was found unresponsive.
